3 hoursClinicians, SupervisorsLive, on-site · Live, virtual · Recorded
Overview
Good notes protect clients, clinicians and the organization. Most clinicians were never taught how to write them, so they learn by copying whoever came before.
This session gives your team one simple structure for notes that are accurate, defensible and quick to finish. Notes another clinician could pick up tomorrow and know exactly where things stand.
Who it’s for
Therapists, counselors, case managers and peer staff who write clinical notes. Supervisors and clinical directors who review them.
Works well for mixed groups of new and experienced staff. Up to 40 people on-site, or 100 virtually.
What your team will learnSample outline. Final content is confirmed in your proposal.
1Explain what a clinical note is for: continuity of care, billing and the legal record.2Write progress notes using a consistent structure such as DAP, SOAP or BIRP.3Link each note to the treatment plan and to medical necessity.4Document risk, safety planning and care coordination clearly.5Avoid the most common audit findings: vague language, copy-forward and late entries.6Finish notes during or right after the session, not at the end of the week.
